Almaaz Membership Terms

This is a translation for the convenience of English-speaking members. If the versions differ, the Portuguese version prevails.

Provider: Clínica Almaaz Serviços Médicos Ltda., trading as Clínica Almaaz, CNPJ 21.590.169/0001-19, Av. Ataulfo de Paiva, 391, suites 407 and 408, Leblon, Rio de Janeiro, RJ, 22440-032, Brazil. Medical director: Dr. Fernanda Pedrosa Richards, CRM-RJ 103617-3, RQE 28908.

Member: the individual aged 18 or over identified in the sign-up record.

1. What the membership is.

• Almaaz Membership is a monthly IV therapy subscription, with two plans: Almaaz Ritual and Almaaz Ritual Duo. The Almaaz Medical Add-on is optional.
• It is a prepaid package of defined services provided by Clínica Almaaz. It is not health insurance or a discount card. It has no risk coverage, no provider network, no emergency care and no expense reimbursement.
• IVs follow a standard protocol set by the medical director. Each IV depends on a technical assessment at the nurse screening: IVs are not sold as off-the-shelf products. The membership does not replace a medical consultation.

2. Plans and prices.

• Almaaz Ritual: 1 IV and 1 booster per month, R$ 1,290/month.
• Almaaz Ritual Duo: 2 IVs and 2 boosters per month, R$ 2,400/month.
• IVs and boosters: the member chooses from the 6 Almaaz IVs (Energy, Immunity, Sculpt, Performance, Recovery and Glow). The booster is B Complex, vitamin D, glutathione or Amino Power, given at the same visit as the IV.
• Almaaz Medical Add-on (optional, only with a plan): R$ 300/month for 2 medical consultations per year, plus an asynchronous review of lab results on WhatsApp, answered within 3 business days. The review is not an emergency channel.

3. Credits.

• Validity: each paid month gives the plan’s credits, valid for 60 days from the billing date. Unused credits then expire without refund. If the member cannot use them for a health reason, with a medical certificate, validity is extended for that period.
• Personal use: credits are personal and non-transferable, except for the guest benefit.
• Booking and screening: every session requires booking and a nurse screening. If the screening advises against the IV that day, the credit is kept. The member may rebook or switch to another IV.
• Late cancellations and lateness: cancelling less than 24 hours before a session, not showing up, or arriving more than 15 minutes late without notice counts as a used credit. The same rule applies to Medical Add-on consultations.
• Several credits in one visit: using more than one credit in the same visit requires prior authorisation from the medical and nursing team at the screening.
• Unused consultations: consultations not used within each 12-month period expire at the end of that period.
• Clinic-side cancellations: if Almaaz cancels a session or lacks supplies, credit validity is extended.

4. Billing.

• Recurring charge: billing is monthly, by credit card. By joining, the member authorises the recurring charge.
• Price adjustment: prices may be adjusted every 12 months by the IPCA index, with 30 days’ notice. A member who does not agree may cancel at no cost.
• Failed payment: benefits are suspended until payment is made. After 30 days without payment, Almaaz may end the contract and charge any amount due, plus a 2% late fee and 1% monthly interest, applying the early-cancellation rule in section 7 if the minimum term has not been completed.
• Invoices: Almaaz issues an invoice for every charge.

5. Member benefits (the same on both plans):

• a monthly body composition scan;
• priority booking;
• 1 guest every 3 months;
• a complimentary booster in the birthday month;
• 1 Mobile IV visit per year with no travel fee;
• a twice-yearly Progress Check report;
• a 10% member rate on IVs and boosters bought outside the plan, Power products and the Telotest genetic test.

Limits:

• The member rate does not apply to consultations, hormones, the weight and body composition program or anything that requires a prescription.
• It cannot be combined with 4- or 8-session packages.
• Guests must be 18 or over, book in advance, pass the screening, sign the consent form and pay for their own IV at the member rate.
• Mobile IV covers Leblon, Ipanema, Copacabana, São Conrado, Barra da Tijuca, Botafogo, Jardim Botânico, Gávea and Lagoa, subject to availability. Visits outside this area depend on feasibility and carry a travel fee quoted in advance.
• Benefits have no cash value.

6. Minimum term.

• Plans have a 6-month minimum term and the Medical Add-on a 12-month minimum term.
• After that, the contract renews monthly.
• There is no freeze or pause option.

7. Cancellation.

• Cooling-off: the member may withdraw within 7 days of joining and receive a refund. Only the list price of any credits already used is deducted.
• After the minimum term: cancellation needs 30 days’ notice.
• During the minimum term: the member repays only the benefit already received, meaning the difference between the list price in force on the joining date (Annex I) of what was used and the total monthly fees actually paid. There is no other penalty.
• No-cost cancellation: there is no charge for moving to another city (with proof), for a permanent medical contraindication (with a doctor’s certificate) or if Almaaz breaches these terms.

8. Health and safety.

• The member gives accurate health information at every screening and signs the IV therapy consent form before the first session.
• Almaaz does not promise or guarantee results.
• WhatsApp is not an emergency channel. In an emergency, call 192 (SAMU) or go to an emergency unit.
• Almaaz is not liable for reactions caused exclusively by health information the member withheld, gave falsely or gave incompletely.

9. Data. Personal and health data are handled under the Almaaz Membership Privacy Notice and Brazil’s LGPD (Law 13,709/2018).

10. Changes. Almaaz may change these terms with 30 days’ notice. A member who does not agree may cancel at no cost.

11. General.

• Venue: the member’s place of residence.

• Acceptance: the member confirms having read these terms and expressly agrees to the key terms, especially the cancellation rules in section 7. Acceptance is recorded on WhatsApp: Almaaz sends the link to these terms and Annex I, and the member replies “ACEITO” (I accept) and authorises the recurring charge.
